India Launches Nationwide Free HPV Vaccination for Adolescent Girls

What's the News?

The Union Health Ministry is rolling out a nationwide Human Papillomavirus (HPV) vaccination programme targeting 14-year-old girls across India. The initiative provides free, voluntary vaccination at government health facilities to prevent cervical cancer. A high-intensity 90-day campaign will kick off the programme, followed by integration into routine immunisation schedules. The government has secured 2.6 crore doses through Gavi, the Vaccine Alliance, sufficient for two years of implementation.

The Vaccine: Gardasil-4

  • Type: Quadrivalent vaccine (protects against HPV types 16, 18, 6, and 11)
  • Manufacturer: Merck & Co (MSD)
  • Dosage: Single dose (as per WHO 2022 recommendations)
  • Effectiveness: 93–100% against vaccine-covered cancer-causing HPV types
  • Coverage: HPV types 16 and 18 account for over 80% of cervical cancer cases in India

Why single dose?

  • Simplifies delivery and improves compliance.
  • Aligns with global best practices (over 90 countries now use single-dose schedules).
  • Makes large-scale immunisation feasible in resource-constrained settings.

Implementation Framework

Target Population: Approximately 1.15 crore 14-year-old girls across India.

Delivery Points:

  • Ayushman Arogya Mandirs (Primary Health Centres)
  • Community Health Centres
  • Sub-District and District Hospitals
  • Government Medical Colleges

Key Features:

  • Voluntary participation with mandatory parental/guardian consent.
  • U-WIN portal for appointment booking.
  • 24×7 linkage to government health facilities for adverse event management.
  • Cold Chain Points (CCP) at all sites for safe vaccine storage.
  • Trained medical officers at each session for post-vaccination observation.

Cervical Cancer Burden in India

  • Ranking: Second most common cancer among Indian women (after breast cancer).
  • Annual new cases: Nearly 80,000
  • Annual deaths: Over 42,000
  • Global comparison: India reported 1.27 lakh new cases and nearly 80,000 deaths in a single year (Globocan 2022).
  • Cause: Almost all cases result from persistent infection with high-risk HPV types.

Additional Info

Global Context:

  • Over 500 million doses administered worldwide since 2006.
  • More than 150 countries have introduced HPV vaccination in national immunisation schedules.
  • India joins this group with evidence-based single-dose strategy.

Available HPV Vaccines in Private Market:

  • Cervarix (GSK, UK) – Bivalent.
  • Cervavac (Serum Institute of India) – Quadrivalent.
  • Gardasil 4 and Gardasil 9 (Merck) – Quadrivalent and Nonavalent.
  • Gardasil 9 (priced above ₹10,000/dose) offers broader protection but government opted for cost-effective Gardasil 4 given HPV 16/18 dominance in India.

Policy Guidance:

  • National Technical Advisory Group on Immunisation (NTAGI) – Recommends vaccine policy in India
  • WHO Position Paper (2022) – Supports one or two-dose regimens for girls aged 9–14 years

Indian Youth Rank 60th in Global Mental Well-being Study

What’s the News?

  • According to the Global Mind Health in 2025 report released by Sapien Labs, young Indian adults (aged 18–34) are facing a significant mental health crisis, ranking 60th among 84 nations.
  • The study highlights a sharp generational divide, with older Indians reporting much higher levels of mental well-being compared to the youth.
  • The findings emphasize that the decline is a "structural, multi-year generational shift" driven by modern lifestyle factors rather than a temporary post-pandemic effect.

The Global Mind Health Report?

  • Metric Used: The study uses the Mind Health Quotient (MHQ), which aggregates 47 parameters across six dimensions: Mood & Outlook, Social Self, Adaptability & Resilience, Drive & Motivation, Cognition, and Mind-Body Connection.
  • India’s Performance:
    • Young Adults (18–34): Recorded an average MHQ score of 33, placing them in the bottom third of the global rankings.
    • Seniors (55+ years): Scored 96, ranking 49th globally and aligning with functional mental health norms.
  • Mental Functioning: The report identifies a decline in "core mental functioning," which includes the ability to regulate emotions, maintain focus, and build stable relationships, rather than just an increase in clinical diagnoses.
  • Economic Impact: Experts note that MHQ is linearly related to productivity, suggesting that declining mental health among youth could significantly impact India's future economic resilience.

Drivers of Declining Well-being?

  • Digital Addiction: The average age for first smartphone exposure in India is 16.5 years. Early and frequent use is linked to higher mental health risks and reduced peer social capital.
  • Dietary Habits:
    • 44% of young Indians regularly consume Ultra-processed food (UPF), compared to only 11% of those aged 55+.
    • High UPF intake is scientifically associated with diminished emotional and cognitive resilience.
  • Weakened Social Ties: Only 64% of young adults reported being close to their families, a notable drop from 78% among the older generation.
  • Reduced Spirituality: Lower scores in spirituality and sense of purpose among youth were identified as factors reducing their "protective cultural buffer."
